• Beacon Place- A Discharge Resource


Often persons we serve have been discharged from rehabilition hospitals or services years before. Whether someone has been affected by a recent Traumatic Brain Injury or stroke, age related issues or disability we are here to help people through whatever challenges they face. We have services available through the Medicaid Waiver Programs and some paid by no-fault, trusts, self-pay or counseling through Medicare, Managed Care Medicaid or private insurance.

Many of our clients have been affected by Traumatic Brain Injury, stroke, trauma, mental health issues, problems with aging or disability for some time, decades in some cases, and have not been treated for years. Whether it be maintaining someone's level of independence or providing services to help someone improve Beacon Place fills the gap when acute treatment and rehabilitation ends. It is unfortunately too frequent otherwise for someone to lose the gain they made or not proceed as far as they possibly could. C

Individualized, person-centered goals are developed with the client, and, if desired, their family. Services can be coordinated with their physicians and other professionals if desired. Mental Health services, and our developing medical and medication management services are there to fill service gaps as well.

We are striving to provide a comprehensive network of services to optimize our clients progress and eliminate barriers that impede progress or lead to deterioration. We hope to work with you, as a team, with that purpose in mind.

•Beacon Place's Executive Director is a Certified Brain Injury Specialist (CBIS), as well as LCSW, MA, CASAC, CHt, with years  of experience. 
•Beacon Place is staffed by trained, experienced professionals working towards all all staff obtaining their CBIS
•Programs and services are well designed to be flexible to meet  multiple needs and goals.
• Each person gets an individualized plan of care.
• Psychological services available, individual & group
• Medicaid Waiver Services for persons having a Traumatic Brain Injury and Stroke and the Nursing Home Transition and Diversion program for those over 65 and those with a physical disability can be considered for the
•TBI Structured Day Program includes a full day of structured activities. •  Breakfast & lunch provided
•Service Coordination, Positive Behavioral Intervention and Support, Community Integration Services are also available at Beacon Place in addition to other waiver services offered by other providers.
• Medical services are being developed as well.
